All applicants applying for admission to the Minnesota Bar must complete the Online Bar Application.
Affidavits of Good Character must meet the requirements of Rule 4B(4), must be dated, and may either be notarized or include the following language immediately above the signature block: “I declare under penalty of perjury that everything I have stated in this document is true and correct.” For additional information about affidavit requirements, see the Board’s Application Submissions FAQs. The affidavits must contain an actual signature; typed signatures will not be accepted. Samples of both forms are included below.
Applicants must complete all applicable supplemental forms using the Online Bar Application. However, if you need to amend your application after you have submitted it, or have been given permission to submit a paper copy of the application for accessibility reasons, please use the forms below.

Applicant Request for Testing Accommodations – Requests for testing accommodations should be submitted by completing Section 5 of the Online Bar Application. If you were unable to do so or an issue has arisen after applying, a copy of the request form can be found here.
Disability Verification Form – Applicant must take this form to a qualified professional(s) for completion and submission to the Board office.
Certification of Accommodations History – To be completed by an educational institution or testing agency.
